Requests for patient medical records should be submitted in writing to the specific facility where the patient is or has been incarcerated, directed to the attention of Turn Key medical staff, along with a valid HIPAA release authorizing the disclosure of the patient’s protected health information. Records requests are subject to applicable statutory charges for copying expenses.
